Lip lift or hyaluronic acid - what to choose for fuller lips?

This is one of those questions with no single answer for everyone. Fuller lips can be achieved in different ways, but lip lift and lip contouring with hyaluronic acid don't work the same way. One is a surgical procedure that permanently changes the proportions of the vermilion and the area under the nose, while the other lets you quickly add volume, improve contour and hydration without surgery. That's why, before deciding, it's worth answering not only the question "how much," but also "in what way" you want to improve the appearance of your lips.

These are not two procedures that give the same result

Lip contouring with hyaluronic acid involves injecting a preparation into the lips and the surrounding area. This lets us gently enlarge the lips, improve their shape, even out the contour, reduce asymmetry, and at the same time improve hydration and smooth fine lines around the mouth. The effect is visible right away, and the final result settles within a couple of weeks.

Lip lift works differently. This is a plastic surgery procedure in which we remove a small piece of skin below the nose, shortening the philtrum and gently everting the vermilion outward. As a result, the upper lip becomes more visible, and the proportions of the lips and lower face change permanently.

In practice, this means one thing: hyaluronic acid adds volume and improves the quality of the lips, while lip lift works more on the proportions and exposure of the upper lip. So if someone says they want "fuller lips," you first need to establish whether they mean more volume, a better contour, improved hydration, or rather that the upper lip is barely visible and optically "hides" under the nose. This step matters more than the name of the procedure itself.

When is hyaluronic acid usually the better choice?

Hyaluronic acid works best when you want a minimally invasive correction. It's a good solution if you want to subtly enlarge your lips, improve their symmetry, define the contour, smooth their surface, or reduce fine lines around the mouth. For many people it also matters that you can return to daily activities right after the procedure, and the procedure itself usually takes 30 to 60 minutes.

It's also a sensible choice for people who want to see a result without committing to a permanent change. Lip contouring with acid gives an immediate but not permanent result. The effect lasts on average 6 to 12 months, depending on individual predisposition and lifestyle. For some patients this is an advantage, as it allows them to build up the effect gradually or check which direction they want to go in next.

We usually choose hyaluronic acid when the problem concerns mainly the volume and quality of the lips rather than their anatomical proportions. If the lips are naturally fairly well proportioned but lack fullness, hydration or a clearer contour, treatment with acid very often turns out to be the first and sufficient step.

When is it worth considering a lip lift?

We more often recommend a lip lift when the problem isn't that the lips are "too small," but that the upper lip is barely visible due to too great a distance between the nose and the lips. During the consultation we pay attention to exactly this detail, because for some patients it's what most affects how the whole lip area is perceived. The procedure is especially recommended for people with a narrow upper lip, teeth that aren't visible when speaking or smiling, and a distance between the upper lip and the base of the nose of at least 15-20 mm.

This is a solution for people who want to change their proportions permanently and don't want to rely on regular filler top-ups. The procedure itself takes about 45-60 minutes, we perform it under local anesthesia, and the patient goes home the same day it's finished. The first result is visible immediately, the full effect usually after about two weeks, and the scar remains hidden in the natural crease of the skin under the nose.

However, it needs to be said clearly: a lip lift isn't some kind of "permanent hyaluronic acid." It's a surgical operation, so it involves recovery, sutures, and the need for more caution afterward. Swelling may persist for a few days, and during healing we recommend limiting facial expressions and following a liquid diet. This is an important distinction when deciding. If you want a quick correction without surgery, acid tends to be the better choice. If you want to permanently improve the proportions of the upper lip, we more often think about a lip lift.

What to choose if you want a natural effect?

Here the good news is that both solutions can give a natural result. With hyaluronic acid, naturalness comes from precise placement of the preparation and moderate shaping. With a lip lift, it comes from a well-chosen correction of the philtrum length and a gentle lift of the vermilion. In other words, naturalness isn't determined by the method itself, but by whether it's well matched to the patient's facial anatomy and expectations.

So if you're mainly thinking about "a fresher, more defined lip look," we often start with less invasive solutions. But if you've long noticed that your upper lip is optically too long, your teeth barely show when you smile, and lip contour alone doesn't solve the problem, it's worth talking about a lip lift.

Or maybe a combination of both methods?

Yes, such a combination is also possible. A lip lift can be combined with, among other things, lip augmentation with hyaluronic acid, but we always make this decision individually. Not everyone needs both procedures. Sometimes improving the proportions alone already gives a very satisfying result, and sometimes, once healed, it's worth further refining the volume or contour.

How to make a good decision?

The simplest answer is: start with a consultation, not with the assumption that "I definitely need acid" or "I definitely want surgery." With lips, the differences can be subtle, but they're exactly what determines the final result. During the consultation we assess not only the lips themselves, but also the proportions of the lower face, the length of the philtrum, tooth visibility and overall facial harmony. Only then can we honestly say whether an aesthetic-medicine treatment or a plastic-surgery procedure would be better.

So if you're wondering what to choose, the answer isn't "always lip lift" or "always hyaluronic acid." The answer is: it depends on whether you want to add volume to your lips, improve their quality and contour, or permanently change the proportions of the upper lip. And that's exactly where it's best to start the conversation.
